Dear Vendor,

We strongly request to provide us with serial number and other relative information, using the following instructions:

For each P.O. please download from the following links the 2 excel worksheets named "Serial Number Template" and "Mac Address Template".

• Serial Number Template .xls format
• Mac Address Template .xls format

On your computer, fill in both excel spreadsheet and save them with the relevant information:

Serial numbers per Product code (end items and sub assemblies), HW version, SW version, warranty start date/ duration and Mac addresses per product code and SN were applicable.
